man pages: Rationalize copyright information.

Thanks to Werner Lemberg, Ingo Schwarze, and Ralph Corderoy for their
feedback and support.

* Stop defining page-local "co" macros to store author information.

* Instead, move this information into comments, removing all roff markup
  (such as character escapes and UR/UE macro usage).

As one might expect, some warts were revealed:

* A couple of people mixed authorial information into the copyright
  notice and license statement.
  + I consolidated Bernd Warken's rewrite claim on groff(1) into the
    existing AUTHORS section.
  + I moved Bernd's claim of authorship of simple examples in tbl(1)
    into a comment separate from the copyright/license info.  If desired
    we can add an AUTHORS section to this page to accommodate this
    information.
  + Francisco Andrés Verdú made an unclear claim of modification in
    grolbp(1).  It might mean that grolbp itself was derived from some
    other output driver.  I didn't dig into this, and simply moved this
    claim into a comment below the NAME section.

* The gxditview man page was missing a license notice that it really
  should have had.  The MIT/X11 license is clear that its permissions
  are granted only "provided that the above copyright notice appear in
  all copies and that both that copyright notice and this permission
  notice appear in supporting documentation".  I went back and looked at
  xditview from X11R5 (1991).  MIT left their copyright notice out of
  the man page itself (the page looks much the same today).  They're
  allowed to do that but their downstreams aren't, technically.  Even if
  the first prong of the fork doesn't bind licensees to correct MIT's
  own omission, the second, "supporting documentation", probably does.

  Long story short: I copied the copyright and permission notices out of
  groff's copy of xditview.c.  This bumped the copyright year from 1989
  to 1991.
