* gthr-dce.h, gthr-posix.h, gthr-posix95.h, gthr-solaris.h,
gthr-tpf.h: Define __gthrw. For all identifiers that might
be weak, introduce weakrefs or non-weak aliases with __gthrw,
and prefix all uses with __ghtrw.
