Improve progress notifications.

- Make command-line progress observer work correctly with an increment
!= 1.
- Allow artifact visitor to visit more than one type of artifact.
- Speed up visiting by caching elements.
- Take effort for scanning and moc'ing into account.
- Give different weights to different types of artifacts, so that e.g. a
linking step will take up more space in the progress bar than a compile
step.
