combine all into a single index

* doc/grep.texi (Index): Merge in Concept Index node,
to have only one index.  Arrange @syncodeindex's accordingly.
* doc/grep.texi (@copying): use @copying; move @contents to
  beginning; rearrange other frontmatter in the conventional ways.
