cmd/addr2line: reimplement in Go
We never updated libmach for the new object file format, so it the existing 'go tool addr2line' is broken. Reimplement in Go to fix. LGTM=r R=golang-codereviews, r CC=golang-codereviews https://codereview.appspot.com/66020043
