initial symbolics and scaffolding
- only use one context for speed. See
https://gitlab.gnome.org/GNOME/adwaita-icon-theme/-/issues/73
- keep some index.theme scaffolding for legacy icons possibly installed
by gnome-themes-extra.

- leave cursors intact, hopefully we'll never touch that ever again
- FIXME: fullcolor. So far we only have app icon preview that does
one icon. Might need special casing for folders, but everything
else seems like doable.
