Improve indexing of modifier keys
* doc/emacs/commands.texi (User Input): Add index entry for
the Alt key serving as Meta.
* doc/emacs/custom.texi (Modifier Keys): Add index entries for
Alt, Super, and Hyper modifier keys. (Bug#38315)
