            increases code size of tiny Vim by only 40 Kbyte.

Solution:   Expand the macro.

            enabled and only adds 7% to the binary size of the tiny build.

Solution:   Set the attribute to clear with.

Solution:   Clear valid flags when setting the cursor.  Set the topline when
            not in full screen mode.

Problem:    Some macros are in lower case, which can be confusing.
Solution:   Make a few lower case macros upper case.

Problem:    Accidentally removed #ifdef.
