Remove the shuffle and boot comapi call.  This is the last of the
comapi calls left; we should now be able to completely remove the
comapi support.

The special handling of serial ports 0-3 meaning "look in a BIOS
table" is at least officially BIOS-specific, so create an inline
function and move it to bios.h.

While we are at it, make the function look slightly less like
converted assembly.

In preparation for strictly limiting the list of variables that are
exported (global) in the core, delete any unused variables, rewrite
variable declarations in C that are not referenced by asm and delete
files that are no longer included in other asm files.

There's no reason to use the COMBOOT API at all now that we can have
any undefined symbols resolved at runtime - we can just access
functions directly.

The syslinux_force_text_mode() function used in library
space made unnecessary INT 0x22 call that could be simply
used with vgaclearmode() function.

This patche renames vgaclearmode() to
syslinux_force_text_mode() as this naming is generally more
carefully considered and gets rid of "forcetext.c" source
file.

Also all the VGA-related functions and variables that were
exported in core/include/bios.h have been moved out to
core/include/graphics.h which makes more sense actually.
