Update the memory layout specification

Update the memory layout specification.  Remove NASM 0.98.38 bug
workarounds, rebalance the bss sections, and introduce the new section
.adv which contain the auxilliary data vector.

The functionality needs some more polish, but we don't actually need

Add subroutines to add/remove blocks from the expiry chain.  This allows
a user to remove a block from expiration and thus hold on to the buffer
for some time.
