Instead of having information in setup.py and in README.rst we now just
have it in README.rst and remove it from setup.py, read it in when we
call setup.py. Simpler.

Fixup parts of the reference documentation to link

We want the reference document to correctly link to the correct location
for the various bits of documentation. This should help users find the
right location in the full documentation.
