roll back the transaction when a flush error occurred, instead
raising its own exception while leaving the session still

- consolidate state assertions in session transaction, use just one
method
- add more unit tests for these assertions

in this system, we use kind of an ugly hack to get the noseplugin imported outside of the
"sqlalchemy" package, while still making it available within sqlalchemy for usage by
third party libraries.

    An "overflow" counter of 100 is also in place,
    in the event of a broken after_flush() hook
