Implementation of [#1546 (BitBucket)](https://bitbucket.org/zzzeek/sqlalchemy/issues/1546).

Very unfinished but I have reflection and setting after table creation working for PostgreSQL.

If you're not opposed, perhaps best to continue discussion on Gerrit?

Support `postgresql_concurrently` on index dropping.

Also adds version detection so that DROP INDEX CONCURRENTLY
only renders if the Postgresql version is 9.2 or higher;
for CREATE INDEX CONCURRENTLY, version detection is added
to allow the phrase to omit if the Postgresql version is
less than 8.2.

None / True / False render as literals.
For SQLite, "IS" is used as SQLite lacks
"IS DISTINCT FROM" but its "IS" operator acts
this way for NULL.

Refine PG inspection methods

This refines get_schema_names(), get_table_names(),
get_view_defintion(), _get_foreign_table_names(),
and get_view_names() to use better queries and
remove unnecessary explicit encoding logic.
