* Definition list instead of tables for footnotes and citations,
  option-list, docinfo.
* Join adjacent footnotes and citation entries.
* Include field lists (in addition to ordered and unordered lists)
  in the test if a list is "simple"  (cf. the html4css1.HTMLTranslator).
* Improve and complete the CSS rules.
* Test documents.
