Fixed several hack classes build to simulate file descriptors

Now there is a single class, StdSim in utils.py, which is intended to simulate stdout and stderr file objects.

This class replaced the following:
- pyscript_bridge.py::CopyStream
- transcript.py::OutputTrap
- conftest.py::StdOut

ACHelpFormatter now inherits from argparse.RawTextHelpFormatter to make it easier to format help/description text

Some fixes to autocompleter to make it easier to do delimited and file completion. Saving state - more to come.

Changed cmd2 to use autocompleter by default for all argparse commands. Not all tests are passing yet.
