src: include uv.h in node_binding header

This was removed in https://github.com/nodejs/node/commit/3bb085dbad4d23030c731834e35b2804c8a50b23
but is needed by Electron on Windows, as otherwise compilation errors
will occur as a result of unknown libuv types.

src: cleanup unused headers

Node codebase has evolved a lot in the more than 10 years of its
existence. As more features (and code) have been added, changed,
removed, it's sometimes hard to keep track of what gets used and what
not.

This commits attempts to clean some of those potentially left-over
headers using suggestions from  include-what-you-use

(Re-?)add a public API for creating linked bindings (access to
`NM_F_LINKED` as a constant was previously removed in
d6ac8a4db0c0a588258f594dc21fbd8018bef7c2), and add a test for
the functionality.

worker: improve integration with native addons

Allow loading add-ons from multiple Node.js instances if they are
declared context-aware; in particular, this applies to N-API addons.

Also, plug a memory leak that occurred when registering N-API addons.

src: unload addons when environment quits

when the environment is destroyed.

src: move C++ binding/addon related code into node_binding{.h, .cc}

This patch:

- Moves the C++ binding/addon related code out of
  node_internals.h/node.cc and into dedicated files
  node_binding.h/node_binding.cc, and only puts the code resued
  by other files into the header.
- Introduce a node::binding namespace so that code exposed to
  other files can be easily recognized.
