getinfo: add microsecond precise timers for various intervals

Provide a set of new timers that return the time intervals using integer
number of microseconds instead of floats.

The new info names are as following:

CURLINFO_APPCONNECT_TIME_T
CURLINFO_CONNECT_TIME_T
CURLINFO_NAMELOOKUP_TIME_T
CURLINFO_PRETRANSFER_TIME_T
CURLINFO_REDIRECT_TIME_T
CURLINFO_STARTTRANSFER_TIME_T
CURLINFO_TOTAL_TIME_T

test 500: workaround low timer resolution on Windows

Since the timer resolution is lower, there are actually cases that
the compared values are equal. Therefore we check for previous
timestamps being greater than the current one instead.

Revert changes relative to lib/*.[ch] recent renaming

This reverts renaming and usage of lib/*.h header files done
28-12-2012, reverting 2 commits:

  f871de0... build: make use of 76 lib/*.h renamed files
  ffd8e12... build: rename 76 lib/*.h files

This also reverts removal of redundant include guard (redundant thanks
to changes in above commits) done 2-12-2013, reverting 1 commit:
