curl: add variables to --write-out

In particular, these ones can help a user to create its own error
message when one or transfers fail.

writeout: add 'onerror', 'url', 'urlnum', 'exitcode', 'errormsg'

onerror - lets a user only show the rest on non-zero exit codes

url - the input URL used for this transfer

urlnum - the numerical URL counter (0 indexed) for this transfer

exitcode - the numerical exit code for the transfer

errormsg - obvious

curl: remove 'config' field from OutStruct

As it was just unnecessary duplicated information already stored in the
'per_transfer' struct and that's around mostly anyway.

The duplicated pointer caused problems when the code flow was aborted
before the dupe was filled in and could cause a NULL pointer access.

It isn't always clear to the user which options that cause the HTTP
methods to conflict so by spelling them out it should hopefully be
easier to understand why curl complains.

tool_cfgable: Renamed Configurable structure to OperationConfig
